Use direct patient quotes in the Subjective section when they capture important clinical details. "The pain wakes me up at 3 AM every night" is more clinically specific than "Patient reports nighttime pain." Quoting the patient also strengthens the note's legal defensibility.
O — Objective
The Objective section documents what you observe, measure, and test. This is clinician-generated data — the findings from your physical examination and diagnostic results.
What to include:

A vague plan like "Continue current medications, follow up PRN" may not support the code billed. Always document the medical decision-making that justifies your management choices.
